Your student needs to have a Medical Release form AND a Damage Waiver/Media Release form on file in order to attend the event. Both of these forms are valid for one year from the date that you complete the forms. When we receive the email confirmation that your student has registered for the event, we will send you an email regarding the status of your student’s forms.
All forms are due by Thursday, February 4th. You can turn them in at the church office, at any InsideOut, or mail them to the church you attending:
