Your student needs to have a Medical Release form AND a Damage Waiver/Media Release form on file in order to attend an event. Both of the forms are valid for one year from the date that you complete them. When we receive the email confirmation that your student has registered for an event, we will send you an email regarding the status of your student’s forms. Please email insideoutevents@northpoint.org with any questions regarding these forms.
You can turn the forms in one of three ways:
1) Bring them to the church office through the West Entrance of the building
2) Have your student bring them to any InsideOut prior to the event
3) Mail them to the church:
